EBSCO changed the search interface for all of its databases, including CINAHL, in July, 2008. Not to worry, however. This new one is easier to use and has more functionality. Call the library (415-206-3114) or use the "Ask a Librarian" button on the database screen if you are having trouble with your search, if you'd like to attend a class, or if you'd like some individual coaching to learn to use the resource more efficiently.

Wikis: What and Why

The term "wiki" originates from the Hawaiian "wee kee wee kee" which means quickly. (Hint: think of the wiki wiki bus at the Honolulu airport that whisks you from the gate to baggage claim) Most people know about the format because of Wikipedia, an open-source encyclopedia that began in 2001 and today contains entries for more than 500,000 topics.

A wiki is a web-based system which supports community authoring. Collaboration and document sharing are some of its most frequently mentioned advantages. Other considerations are:

Opportunity to pool expertise, benefit from the group's collective wisdom, and generate synergy and creative ideas.
Efficient way to do document editing where several people need to give input.
Easy to administer and to add various types of content including PowerPoint presentations, audio and video podcasts, calendars,surveys, photos, and widgets of all varieties.

There is concern about security, but most wikis (including this one) can be either public or private, where invitations to join are sent to specific individuals. It's also possible to grant page permissions within the wiki, with individuals being granted access to only certain pages.

4B Dysphagia EBP Project

Welcome to the 4B Dysphagia program page at the EBP @ SFGH Wiki. I'm thrilled to share with all of you the latest news about our program.

We have conducted 2 training sessions on Dysphagia in 4B (Neuro/Stepdown Unit). First, Speech Language Pathology (SLP) and Nutrition joined me during the Dysphagia Awareness training to review key-concepts of dysphagia and evidence-related interventions for patients at risk for Aspiration / Dysphagia.

Currently, we are bringing the theory to practice by conducting the second installment of training: the Swallowing Screening Test for nurses and the Aspiration Pneumonia STOP Campaign.

This project is having a great reception from all nursing staff in 4B and from other departments within SFGH (e.g. SLP, Nutrition, Medicine). I'mthankful for the willingness and commitment 4B nurses have shown to this project. Once we start rolling out the standardized nurse-led Swallowing Screening Tool, I'm certain not only Aspiration PNA rates will decrease in patients @ risk but also nourishment should improve in patients that not show swallowing difficulties from the screening process.

Loansome Doc is the document delivery service that's built into PubMed. You know how frustrating it is to need the full text of an article and keep getting asked for user name and password, or worse yet, get asked for $25.00? The Barnett-Briggs Medical Library at San Francisco General Hospital makes it easy for you to get those needed articles, and at no cost to you if you're a DPH employee based at SFGH.

Here's a tutorial that explains how to sign up. You'll need to have a computer with audio capability and speakers. If you don't, call us at 415-206-3114 and we'll walk you through the registration process.

AHRQ Evidence Based Practice - Links to evidence-based practice Centers (EPC) Evidence Reports.

Best Practice Guidelines - Developed by Registered Nurses Association of Ontario (RNAO) as part of their nursing best practice guidelines project begun in 1999. Focuses on the nursing profession, but also includes clinically- related guidelines.

Best Practice Information Sheets - Provides access to PDFs of best practice guidelines produced by the Joanna Briggs Institute of Australia. Not as comprehensive as some sites. Publishes approximately 4 guidelines per year.

Evidence-Based Medicine Resource Center - New York Academy of Medicine links to clinical practice guidelines.

National Guideline Clearinghouse(NCG) - Produced by the AHRQ (Agency for Healthcare Research and Quality). Includes evidence-based clinical practice guidelines. Includes guidelines focused on prevention, diagnosis, treatment, education, risk assessment, etc. Advanced search works best.

National Institute for Health and Clinical Excellence (NICE) A UK-based organization that develops and publishes "guidance" in the areas of health technologies and clinical practice.

US Preventive Service Task Force Recommendations Convened by the Public Health Service to rigorously evaluate clinical research assessing the merits of preventive measures, including screening tests, counseling, immunizations, and preventive medications, this group publishes its recommendations as part of the AHRQ web site.

Automated Current Awareness

Most bibliographic databases have the capability to provide you with citations to newly-added publications on subjects of interest to you. PubMed is no exception. To get started with automated current awareness, take a look at the information put together by UCSF librarians. On that page, there's also a link to an automated online tutorial developed by the National Network of Libraries of Medicine that's worth looking at. It takes only about 4 minutes.

Attached below is a PowerPoint currently being presented to patients as part of a research project involving HIV+ and AIDs patients. The objective is to determine if a one-hour basic computer skills and Internet search and evaluation class improves outcomes for patients as measured by improved ability to find authoritative disease and treatment information on the Internet and better self management as measured by number of Emergency Department visits.

Evidence-Based Behavioral Practice is a web-based training resource currently under development. It is funded by NIH's Office of Behavioral and Social Sciences Research and aims to bring evidence-based practice to the behavioral and social sciences, a sector of health care that is a relative newcomer to EBM.

Best Bets
BETs were developed in the Emergency Department of Manchester Royal Infirmary, UK, to provide rapid evidence-based answers to real-life clinical questions, using a systematic approach to reviewing the literature. Although BETs initially had an emergency medicine focus, there are a significant number of BETs covering cardiothoracics, nursing, primary care and pediatrics. Search capability is not sophisticated, but if you happen to find a review on your topic, you can save yourself a lot of time.

Contains nursing literature as well as literature from many other biomedical fields.
Use the nursing subset on the Limits page to restrict to nursing journals.
Many MeSH (medical subject headings) allow you to apply a /nursing subheading to get at nursing-related ideas.

“Clinical queries” makes it easy to find evidence-based information. (pre-formulated search filters for diagnosis, therapy, etiology and prognosis questions)

For full text access, use PubMed via a UCSF networked computer.
Freely available at www.pubmed.gov, but no full text, only abstracts when available.

Tutorial Good overview of features and functions. Takes a while (30-45 minutes) but is worth the trouble.

Evidence-based Practice at SFGH

Here's a small collection of online evidence-based practice resources. This site is in Wiki format and is accessbile on the open web at http://ebpsfgh.wetpaint.com. It was developed by Librarians Joy Graham and Terri Mason at SFGH's Barnett-Briggs Medical Library as a place to park helpful information, such as class handouts, and resource descriptions. We created it hoping that you would add content that you find helpful.

We've chosen the sites we think are most valuable and useful for nurses and other clinicians, but we've just begun and welcome your ideas for additions. If you'd like to add content yourself, please apply to be a writer.

Compiled by the Cochrane Collaboration, composed of a experts worldwide in specific clinical areas of interest.
Source for systematic, high-quality, up-to-date reviews of health-related topics. Currently has 2250 full text reviews that are updated continuously.
Also includes unpublished data and much non-English data.
Consists of seven parts, but two are most useful to clinicians: Database of Systematic Reviews, Database of Abstracts of Reviews of Effects (DARE),
Many of systematic reviews included in CINAHL and PubMed.
Uses MeSH headings and subheadings.
Has History, so you can combine prior search statement retrieval.
Can save or edit your searches.
